HDC Small Group Learning Session: Adult Mental Health

  • Overview

Please note this session is open for Family Physicians and Nurse Practitioners of the Central Island Division Region only.

This two-hour small group session aims to provide learners with essential knowledge and practical approaches to enable timely implementation of best practices, guideline-informed care and practice-ready resources regarding Adult Mental Health into their workflows.

Local facilitators will lead the session and case review to allow communities of practice peers to explore unique needs, based on the Health Data Coalition (HDC) Discover aggregate community and participating individual practice data where available. This real-time community data may help focus potential solutions, as well as foster pivotal relationships amongst peers, develop a common understanding of the key issues presenting to clinicians, and standardize approaches for clinicians to effectively utilize the resources available.
