Overview

This learning module is primarily about safety.

Effective communication & documentation leads to best health outcomes for our clients and protects the provider and other members of the team.

Communication has been identified as a leading source of medical error in Canada.

It is critical that midwives communicate effectively within the team.

This module reviews strategies for enhancing effective communication through our interactions:

  • Verbally with others; and
  • Through a legal written record of care.

We will review approaches to both spoken and written communication and the legal standards related to legislation.

Outline

This course is divided into four lessons:

  1. Introduction: This section highlights the role of communication and documentation in supporting best care outcomes.
  2. Regulations and the Law: This sections reviews the statutes and policies that dictate communication and documentation standards.
  3. Communication and Teamwork: This section provides tools to support effective communication.
  4. Documenting Care: This section reviews the purpose of documentation, the role of the intellectual footprint, the six essential elements of the records and how ensure the medical record reflects credible and defensible practice.

Undergraduates and Bridging Students in UBC Midwifery ProgramFree (included in tuition)
Midwives outside BC and other Healthcare Professionals$119 + GST

Learning objectives

By the end of this course, you will be able to:

  • Describe the legal, regulatory and professional requirements of midwives as related to documentation and the medical record.
  • Discuss the role of communication in client care outcomes.
  • Identify effective communication strategies for ensuring client safety.
  • Describe the role and content of handover tools.
  • Describe the following documentation tools and their role within medical records.
  • Identify a variety of midwife-client encounters and document appropriately leaving an intellectual footprint.
  • Discuss the impact of technology on documentation and incorporate safe standards for maintaining appropriate medical records.
